MDS의 진단기준, cytopenia와 dyplasia의 기준, genetic abnormality에 대해 알아보자.

2. Cytopenia and dysplasia 

•  CCUS, MDS, MDS/MPN 에 적용되는 cytopenia 기준은 모두 동일하다. (5th WHO classification기준)

(1)   Hb <13 g/dL in males and <12 g/dL in females for anaemia

(2)   Absolute neutrophil count <1.8 ×109 /L for leukopenia

(3)   platelets <150 ×109/L for thrombocytopenia

 

• Threshold for dysplasia

 (1) 10% for all lineages

 (2) Dysplasia 를 보이는 linega의 개수는 MDS진단기준에서 이제 사라졌다.

3. MDS with defining genetic abnormalities and MDS, morphologically defined

• MDS with low blasts and SF3B1 mutation (MDS-SF3B1)

Ring sideroblast가 15% 이상 보이면 SF3B1 mutation이 없어도 MDS-SF3B1 mutation있는 것으로 간주한다.

 

• Biallelic TP53 (biTP53) alterations 은 무엇인가?

크게 2가지로 나눌 수 있다.

(1) multiple TP53 hits (multi-hits)

Multiple mutations in TP53

Multi-hit 돌연변이는 residual wild-type p53 protein 이 결여된 neoplastic clone을 초래한다.

(2) biallelic TP53 alterations

Mutation with concurrent deletion of the other allele

 

• Revised IPSS-R (Revised International Prognostic Scoring System for MDS)

Scoring의 목적은 치료시작전 성인 MDS환자의 예후를 평가하기 위함이다.

Revised IPSS-R calculator 를 인터넷으로 검색하면 calculator를 이용할 수 있다.

 

• MDS-biTP53

환자들의 90% 이상이 complex karyotype을 갖고 있다. 그래서 이 환자들은 Revised IPSS-R 로 계산하면 very high risk category에 해당된다.

 

• MDS-h, PNH (Paroxysmal nocturnal haemoglobinuria), AA (Aplastic anemia)

3가지 질환은 triad 로 불릴 정도로 겹치는 특징들이 있고, 질병들이 같이 동반되는 경우가 많다.
