Clonal haematopoiesis-CHIP,CCUS

1)   CH (Clonal haematopoiesis)

 CH는 precursor myeloid disease state에 해당하는 category다. 따라서, CH(CHIP, CCUS)의 진단은 haematologic maglinancy, myeloid neoplasm 진단을 받지 않은 상태에서 이루어져야 한다.

 

CH에는 *CHIP 과 **CCUS 두 가지가 있다.

*CHIP = Clonal haematopoiesis of indeterminate potential

**CCUS = Clonal cytopenia of undetermined significance

CHIP은 myeloid malignancy-associated somatic mutation 이 있는 상태이며, CCUS는 CHIP상태에서 cytopenia가 동반된 경우다.

 

여기서 CHIP의 myeloid malignancy-associated somatic mutation 기준과 CCUS의 cytopenia 기준을 살펴보자.

2)       CHIP의 myeloid malignancy-associated somatic mutation 기준

Somatic mutations of genes detected in blood or bone marrow at VAF ≥ 2%

(VAF ≥ 4% for X-linked gene mutations in males)

3)       CCUS의 cytopenia 기준

CCUS, MDS, MDS/MPN의 cytopenia 기준은 동일하다.
Hb	< 13 g/dL in male	anemia
	< 12 g/dL in female
Absolute neutrophil count	< 1.8*109/L	leukopenia
platelets	< 150*109/L	thrombocytopenia
