말초혈액도말에서 blast의 특징 5가지에 대하여 알아보자.

1)Blast 란 무엇인가?

Blast는 abnormal Immature blood cell 입니다. 좀 더 자세히 설명하면 immature WBC이며, 그 종류로는 myeloblast, lymphoblast, monoblast, promonoblast 등이 있습니다. 건강인에서는 골수(bone marrow)에서 5% 이내로 관찰됩니다.

 

2)Blast의 morphological features

5가지 정도로 요약해보았습니다.

(1)cell size가 medium ~ large로 큰 편입니다.

대체로 lymphocyte (7–10 µm in diameter) 보다 크기가 큽니다. 그리고 작다고 해도 대부분의 blast가 monocyte (16-22 µm in diameter)와 크기가 비슷합니다.

 

(2)대체로 N:C ratio (nucleus-to-cytoplsmic ratio) 가 대략 4:1 정도로 보이는 경우가 많습니다.

즉, large nucleus with scant cytoplasm 형태를 보이는 경우가 흔합니다. 

 

(3)Immature chromatin 이 마치 미세한 점들이 모인 것처럼 관찰됩니다. 

참고로 정상세포인 monocyte의 chromatin은 어둡고 얼룩진 연필선 처럼 보이는 경향이 있으며  정상세포인 lymphocyte의 chromatin은 두꺼운 크레파스로 칠한 듯한 모양을 띱니다.

 

(4)Blast는 대부분 granule 이 없는 경우가 많습니다. 하지만 APL (Acute promyelocytic leukemia)는 예외입니다.

 

(5)Auer rods 는 APL 의 blast에서 보이는 orange-pink, needle-like granule 입니다.

(3)Blast와 hematogone의 차이점

Hematogones(HGs)은 benign lymphoid precursor 입니다. 생긴 모양이 blast와 유사하지만 “benign” 이라는 게 차이점입니다. 모양만 보면 hematogone은 ALL의 lymphoblast와 구분하기 어려운 경우가 흔합니다.

Hematogone은 골수(bone marrow)에 있는 “normal” b-lineage lymphoid precursor cell 이며 3가지 stage – stage 1,2,3 가 있습니다.

제가 “normal” 을 강조한 이유는 hematogone 은 ALL의 lymphoblast와 달리 pathologic 하지 않기 때문입니다. 따라서 hematogone과 lymphoblast를 구분하기 위해 immunephenotype을 확인합니다. (immunophenotype은 direct fluorescence measurement using multicolor flow cytometry 검사방법을 이용해 확인합니다.)

 

또한, hematogone은 type에 따라 나타나는 immuphotype 이 다릅니다.

말초혈액에서 stage 1 과 stage 2 hematogones 이 정상적으로 관찰 될 수 있으며 이 경우 lymphoblast 와 구분하기 위하여 flow cytometry 검사를 시행할 필요가 있습니다.
